lcaron and ldot combine components differently:
in the case of lcaron the RSB uses that of the ‘l’
in the case of ldot the RSB uses that of the ‘periodcentered’
It makes sense but how is this implemented and can it be changed without deactivating the automatic alignment ?

That’s because the caroncomb.alt is a nonspacing mark, while the periodcentered (.loclCAT) is not.
You can use (=)=+x for (layer) glyph-wide x additional spacing.
I usually have something like ==+40 in lcaron/dcaron for +40 RSB on a layer, depending on the layer’s design.
